WAIVERS AND AGREEMENTS
The following waivers were acknowledged during online registration. This document is for registrants to have a permanent record of the policies and expectations agreed to online.
For the following waivers and agreements, the term “registrant” refers to the person(s) who will be taking part in the class, activity, performance, and/or event.
• If the registrant is under the age of 18, the person agreeing to these waivers and agreements verifies they are the legal parent/guardian of the registrant and have the legal authority to consent on their behalf.
• Registrants 18 and older should complete these waivers and agreements themselves. Consent to Participate and Waiver of Liability
I consent for the registrant to participate in the audition for Cincinnati Ballet’s Company or Second Company. I understand dance requires sustained, repetitive, vigorous physical activity, usually performed on a wood floor or lightly padded surface without protective footwear (i.e., athletic shoes). I acknowledge there are inherent risks of serious injury in all the above activities as well as in the general participation of Cincinnati Ballet activities and events.
I shall indemnify, hold harmless and defend Cincinnati Ballet, including its employees, agents and trustees (the "Cincinnati Ballet") against any and all claims, actions, or suits brought for damages or alleged damages, and from all liability, loss, and expense, including reasonable legal expenses, resulting from any injury to person or property or from loss of life sustained by the registrant while participating in auditions, on the premises of Cincinnati Ballet’s Center for Dance and other venues where audition activities may take place.
I understand the registrant is responsible for maintaining control over all personal items and agree I shall not hold Cincinnati Ballet liable for the loss or theft of the registrant’s personal items.
Medical Release
I declare I have either consulted a physician or voluntarily chosen not to consult a physician regarding the registrant’s health before the audition. I have been warned the registrant must be in sufficient health to participate in the audition and now declare the registrant is in sufficient health.
In the event non-emergency medical attention is required for a minor while attending a Cincinnati Ballet audition, the minor’s parents or guardians will be contacted. If the parent or guardian cannot be reached, I authorize Cincinnati Ballet to arrange for the registrant’s treatment as necessary, at my expense.
In the event, emergency/urgent care is required while attending a Cincinnati Ballet audition, Cincinnati Ballet and its employees and representatives are hereby authorized as the agent for the registrant to consent to emergency medical or dental care on their behalf, including x-ray, MRI, and other diagnostic imaging examination, anesthetic, dental, medical, or surgical diagnosis or treatment, or hospital care under the supervision and upon the advice of a licensed physician or dentist. I understand I am solely responsible for any medical, health, or personal injury costs resulting from the registrant’s participation in Cincinnati Ballet auditions.
